What is perimenopause and how does naturopathy help?

Quick answer: Perimenopause is the transitional phase before menopause, typically beginning in your early-to-mid 40s but sometimes as early as 35. Symptoms include hot flushes, night sweats, mood changes, weight gain, sleep disruption, brain fog, and anxiety. Naturopathic support uses targeted herbal medicine, dietary strategies, and nutrient optimisation to ease the transition — either alongside HRT or as an alternative.

Perimenopause is one of the most underserved areas in women’s health. Many women are told to “just wait it out” or offered HRT as the only option. While I fully support clients who choose HRT, there is a great deal that naturopathic support can offer — either alongside hormone therapy or as a standalone approach.

The transition can span five to ten years, and it often catches women off guard. Many don’t realise that perimenopausal symptoms can begin while periods are still regular, and that the anxiety, sleep disruption, or weight gain they’re experiencing has a hormonal basis.

What causes perimenopausal symptoms?

Perimenopause is characterised by fluctuating hormones — oestrogen doesn’t simply decline in a straight line. It swings erratically, sometimes to levels higher than during the reproductive years, before eventually declining. This volatility drives many of the symptoms: hot flushes, night sweats, mood instability, sleep disruption, and brain fog.

Progesterone typically declines before oestrogen, creating a relative oestrogen dominance that contributes to heavier periods, breast tenderness, fluid retention, and irritability. The ratio of these hormones — not just the absolute levels — is what matters clinically.

Research shows that women with higher chronic stress, poorer sleep, and metabolic issues (particularly insulin resistance) experience more severe perimenopausal symptoms (Thurston & Joffe, 2011, Obstetrics and Gynecology Clinics).

What are the most common symptoms you see?

My perimenopausal clients commonly experience hot flushes and night sweats, disrupted sleep (particularly waking between 2–4am), mood changes — anxiety, irritability, or low mood that feels disproportionate, brain fog and difficulty concentrating, weight gain especially around the middle, changes in menstrual pattern (heavier, lighter, or irregular), decreased libido, joint stiffness, and fatigue that doesn’t resolve with rest.

How do you support clients through perimenopause?

Adrenal support: Your adrenal glands take over a degree of oestrogen production post-menopause. If they’re already depleted from chronic stress, the transition is significantly rougher. Supporting adrenal function with adaptogenic herbs is often foundational.

Herbal medicine: Targeted phytoestrogenic and hormone-modulating herbs — including black cohosh, sage, and vitex — have good evidence for reducing hot flushes, improving sleep, and stabilising mood (Franco et al., 2016, Cochrane Database of Systematic Reviews).

Nutrient optimisation: Magnesium for sleep and nervous system support, B vitamins for energy and mood, vitamin D for bone health, and omega-3 fatty acids for inflammation and brain function.

Blood sugar management: Insulin resistance often worsens during perimenopause, driving weight gain and exacerbating hot flushes. Dietary strategies to stabilise blood sugar are a key part of the treatment plan.

Liver support: Healthy oestrogen metabolism depends on liver function. Supporting the liver’s clearance pathways helps manage the hormonal volatility that drives symptoms.
